looking for a form to correct my birthday error i

my birthday on my birth certificate was change from 01-31-1961 to 01-30-1961 this happened when i was on a will from my grandparents then i was told there is no records of the will, what kind of form do I need for correction right now i am on paper work from school my birthday 01-31-2007 years later i have been forced to use 01-30-1961 because someone changed the date there is also issues on my credit report

Go online to the Texas Department of State Health Services. There is a link to file an amendment to your birth certificate. You will need proof. (I do not understand the reason why this was changed, but if there was any deception or fraud involved, you should talk to a lawyer before filing anything).
