If you signed a contract for service and the part they filled out with my name is not my last name. And i signed at the bottom my real name. is this a legal document

Yes, it is. The only way this would not be legal is if this mistake was material, i.e. if they are contracting for Bob Dylan (the artist) and you're Bob Dalan (not the artist) and yet sign it anyway. Otherwise, this is not a material mistake, and you are going to be bound to that contract.
