Original Creditors and Collection Agencies

Can an original creditor or a debt and the collection agency report negatively and simultaneously on my credit report? What are the stipulations?

I believe so, although it should only appear as a single debt, not two separate debts. Now it's possible that the original creditor reported the debt, then sold the negotiable instrument of the debt to a collection agency who now can report the debt as well.
