Legal Question in Credit and Debt Law in Texas

I have been served with a citation to appear in district court. I am being sued for an old credit card debt. I have never had any dealings with the suing entity. I need to file a written response and need to know how to do so (the form it should take)

1. Defendant denies each and every, all and singular, the allegations set forth in Plaintiff's Original Petition and demands strict proof thereof by a preponderance of the credible evidence as is required by the Constitution, statutes, laws, and Rules of Civil Procedure of the State of Texas.

WHEREFORE, PREMISES CONSIDERED, Defendant prays that on final hearing of this cause Plaintiff take nothing; that Defendant have h<> costs; and for all such other and further relief, both at law and in equity, to which Defendant, by this or any subsequent pleading, may show h<>self justly entitled.

And when you file this with the clerk's office, PAY THE JURY FEE. It will be $30 (at most), and it's worth it.
