What will happen?
I have the hospital bill over 10K and paid every month the amount I could afford. Last month the original creditor tried to make a lump sum settlement for 4K. I responded and requested to extend their adjusted offer for several month and continuing to pay by small installments.
Today I have received a Notice of Debt from West Asset Management for original amount 10K.
Is it a third party, Collection Agency? Can I go back to the original creditor? If not, what is the worst will happen?

When you asked them to keep the reduced offer open, that was in effect a counteroffer, rejecting the original offer.
Medical bills are often difficult to collect and the creditor and the collection agency will frequently engage in additional negotiation to resolve the balance.
I do not know if West Asset purchased the debt, or is a collection agency for the original creditor. Ask them, they are required to tell you. You can engage in negotiation with them, and copy your correspondence to the original creditor.
If you have sufficient funds to make a large payment in settlement of this debt, you may consider retaining an attorney to negotiate the settlement.
