Written response to a creditor's citation

a creditor is suing me for nonpayment of debt. I orginially signed up with a consumer counseling service that mistakingly forgot to mention that the creditor had not responded to their proposition, so after 4 years of paying monthly payment, when I called to pay them off, I still owed the original balance. I have to provide a written response to the county clerks office, how do I get the correct form? How should I respond?

You can send a letter to the court, identifying the cause by case number and parties, and state that you deny the claim. Send a copy to creditor's counsel.

Then call the attorney, explain why the debt wasn't paid, and see if you can work out a schedule of payments, or a single lump-sum payment at a fraction of the debt, to compromise and settle the matter.

Send the letter and start negotiations ASAP.
