Legal Question in Criminal Law in Texas

My daughter has surcharges based on tickets received while driving w/o license, however after reading the website : DPS Carrollton Tx., I have learned she can get reduced surcharges if she gets an affidavit showing indigency or poverty level wages. What affidavit form is needed for this?

Per DPS's website: Indigency Program The Department�s indigency program will begin in April 2011. It will apply to individuals at or below 125 percent of the federal poverty level, using a sworn affidavit. The program will reduce the surcharge amount owed to 10 percent of the total, not to exceed $250. It will also stop suspension for those who qualify for the program while making payments. Anyone assessed a surcharge since September 30, 2004 will be eligible.

A law creating a separate indigency program goes into effect September 1, 2011[1]. If a court determines an individual is indigent at the time of conviction, the surcharge will be waived.

I looked all over the site but found no affidavit (which is not surprising since it just became effective last month. However, it does provide a phone number for questions: Municipal Services Bureau at 1-(800)-688-6882. Note that this is the vendor charged with the responsibility of collecting the money.
