Is there a form to file a motion for time served? State case

No. The defense lawyer and the prosecutor discuss the potential punishment on a case and if they agree to time served, then it is written up so the person gets time served. Are you talking about a time cut? if so, forget it unless the person is dying or has cooperated in prosecuting people in prison to his own potential detriment.
