is there a form i can sign allowing a sex offender to be around my kids if they are a family member?

First, if the sex offender is on probation or parole, then they cannot be around kids no matter what you say. Their officer or the court set the rules and they do not care whether there are others - family or outsiders - who would allow the offender around kids. If he is just registering but not "on paper" then you need not give permission.

Second, you are asking for CPS to become involved in your life if you allow a sex offender around your kids.

Third, if you are divorced or not with the children's other parent, you are setting up a situation where the other parent might ask a court to give them custody because of your decision to allow the children around a sex offender.
