why can I not find a form for Mandamus?

I don't know what forms are available online. You should be able to find a shell form at a law library. The body of a mandamus, however, would not be in a form book, as it will track the facts of the case and the law specific to your situation. It would then indicate why the judge has ruled wrongly, and ask the appellate court to order the trial judge to rule correctly. TCL
