The forms you will likely need will be a motion and order for the judge. Call the clerk of the court and tell them that you want an occupational and u don't have a lawyer and ask if they could tell you what you need to do. They probably have forms there at the courthouse you could use. The clerks are normally very helpful, give them a call.
