My brother-in-law is suffering some sort of dementia and has quit eating, bathing, is hallucinating, but flatly refuses to go to the hospital for treatment of ongoing and new problems such as congestive heart failure and infection. We had paramedics out and police when he disappeared for a day but were told that unless he threatens suicide we can't do anything! He desperately needs a secured hospital ward for treatment or he'll walk out, and probably a nursing home. What can we do?

If he is a danger to himself or others because of a mental condition (ie dementia) then he can probably be put into protective custody at a state mental hospital. Try calling 211 for more information. You may also try contacting your local/regional MHMR (mental health mental retardation) center.
You may also consider seeking guardianship protection. Should you decide to do so you need to consult an attorney.
