Trampoline liability

I have just bought a trampoline with enclosure for my children. Is there a form that I could have filled out or signed by the parents of any child that has permission to jump on the trampoline that will keep me from being sued incase of injury while jumping? Possible the ''Permission, medical and Liability Consent Form''?

NO. Anyone can sue for anything. There are forms that would, however, prevent you from being successfully sued. Ask some local lawyer who specializes in personal injury cases to provide you one. Should cost about $100 in legal fees, as he should have a pre-existing form he can whip out and customize for your situation. I can do likewise for the same price if you want. If you are negligent, however, such as having a defective trampoline or not supervising the activity properly, recognize that even the best form may not save you.
