Our 5 year grandson's father must have supervised visititation. This is court ordered. We have found the father alone with the child. Can the courts revoke his rights?

Probably not. The court could fine him for being in contempt of the order.
Revoking parental rights is an extreme measure. Courts are reluctant to do this.
I assume that the mother has an attorney. If not she MUST retain an experienced family law attorney to represent her! This is imparative!
To try to answer your question would be impossible since I was not present & I don't know the facts in your case.
I doubt the judge will "revoke" (terminate) his parental rights.
However, the judge might make the father's visits through the SAFE program or even temporarily end the father's visits until he fulfills certain conditions that the judge determines is in the "child's best interests".
I'd quit wasting your time on the internet asking questions, I'd be in your attorney's office tomorrow! Only an attorney knowledgeable about your case can answer your questions!

I'm not accepting cases since my mom just died & I've temporarily closed my law office.
