adding a.k.a. to given name

My son is the 3rd, having the same 1st, middle and last name as his father and grandfather. To relieve confusion, he has ALWAYS been called a name which is an Irish form of his given name. He doesn't want to change his name---but is it possible to add a.k.a. on legal records so that he can use either name and avoid legal hassels?

No. You are trying to avoid a legal name change by doing it informally. On legal documens that require his legal name, it is not his nickname, but his birth name.
