adding a second middle name

My 14 year old daughter wants to add a second middle name, how do I go about doing this legally?

Assuming both parents are willing to agree to this, you file a lawsuit in family court to change her name. If both parents don't agree, it's not going to happen. Perhaps she should wait until she's an adult to make this kind of decision.
