Adoption by step-father

My 16 year old daughter has not seen her biological father in 9 years. He has agreed to sign papers to let my husband adopt her. Is there a form we could obtain and file it ourselves, or is it necessary to go through an attorney to have my husband (of 10 years) adopt her?

Some of the forms are availaable in the local county law library. I am ethically prohibited from assisting someone to find forms and fill them out. It is much more economical to hire a lawyer who has done this before. Sorry.
