Is there an age limit where a child in Texas can refuse to visitation with a non-custodial parent? Does a person have to be physically attacked in order to get a restraining order? How hard is it to ask for and get supervised visitation for a non-custodial parent?

Technically, there is no age when a child can refuse visitation.

A person does NOT have to be physically attacked in order to get a restraining order.

It is difficult to get a judge to order supervised visitation. You'll need some dramatic facts and some convincing evidence.
