appointing a guardin for minor children

Is there a legal document that can be put in place to

appoint a guardian for--name removed--2 minor children. I was not married to the father of either of--name removed--children, and the fathers are not listed on the birth certificates.

I would like something in place so may parents would

get custody if I am ever incapicated or incarcerated.

Yes, but it will reuire petitioning the court in the county where the children reside, if no other court has current jurisdiction over the children.
