change last name on birthcert.

Im 41years old. all my id's and properties have my fathers last name. I've have used my fathers last name all my life even my social security # has my fathers last name. but my birth certificate has my mothers last name. my father does not appear in my birth cert. i was born in brooklyn ny. how do i fix my last name on my birth cert. to my correct last name?

Contact the NY Vital Statistics office, to determine if they maintain an informal procedure to accomplish this. If not, you can file for an adult name change, which when ordered by the court, will result in the issuance of an amended birth certificate.

You can file suit to change your name.
