Name change for Bride

I have a petition to change my first name (deleting 3 letters) and now I am married and want to hyphenate my husband's name with my last name. How would I put this on a petition for name change. I would like for it to show my new first name which I have always used - along with my last name hyphenated with my husband's last name. What do I put for the reason? Can I do this all on one petition?

Detail the changes on a single petition. A properly prepared petition will contain all names used previously, and the actual name desired. If the petition is pending, then file an amended petition.

Your question states that you are petitioning to change your first name, but now that you want to keep your first name the same, and change your last name? If that is the question, you could have just put that on your marriage certificate.
