First Name Change

The court petition to change your name asks for your legal name and the name you want to change it to.

How do I show that Bobbie and Bobby are to be changed to ''new first name''.

My 1st name on my birth certificate is Bobbie which I never used and not even aware of many yrs. I used Bobby until I was 22, and then I started using ''new first name'' and continue to do so. I am 71 yr old.

My first name on my SS/Medicare records is Bobby and that was not a problem until recently when Medicare stopped paying medical bills for ''new first name''. They did pay them for the previous 6 years.

SS/Medicare will not change my name on their records unless I obtain a court order showing my name(s) were changed to ''new first name''.

SS states that they will only use the name on the birth certificate unless it has been changed in the courts. However, they are using Bobby.

It sounds like you would change your name from Bobby to Bobbi. In a petition, you can asdd the specific reasoning. With the new homeland security issues, you will have to get a set of fingerprints and submit them with the court as well. Have you filed anything yet? In what county will you be filing in?
