Change of Custody

My 12 yr. old daughter is wanting to move home from her Dad's. What is the form that needs to be filled out and signed stating her wishes?

It is called a Preference of Child Twelve Years or Older, and it is just part of what is needed for a modification of custody. Also, remember it just states the child's preference. The Judge is not bound by it.
