Legal Question in Family Law in Texas

We are trying to change an exisisting visitation order to a standard posession order. We would also like to have the child's name (She's 8 and would like this as well) changed to her Dad's last name, she currently has her mother's last name and her mother is getting married in April. What forms would I need to do all of this? The standard posession we are asking for is standard in Texas so we don't feel we will need an attorney even though the mother is contesting this. Thoughts?

Go ahead and get an attorney. If the mother is contesting it, you need one.

Your lawyer would file a motion for modification of the order; the request for the name change for the child could probably be done in the same motion.
