Name Change

What legal actions are needed in order for a person to change their last name if the biological father is not shown on the birth certificate?

Also, what would the total cost be for doing so?

An adult can change thier name by petition to the court with certain affirmations. See a lawyer in your area. The total cost will include attorneys fees, filing fees and court costs.
