Name Change for Minor Child
Who handles an agreed-to name change for a minor child to his legal father's sirname? The middle name will also be changed. The parents were never married. There is no written child support agreement.

Do you mean which parent handles it? Whichever one is willing to pay for and meet with the attorney. Since it is agreed, both parents will sign off on it. A good attorney is going to encourage you both to formalize the arrangement of custody, visitation and child support.
