name change

my question to you, i receive child support, but on my child birth certificate there is no father name, my child has never came in contact with the biological father, i want my child last name to be the same as mine, my question to you, do the biological father has to sign any documents for her last name to be changed.

Re: name change

If there is a court order providing for the biological father to pay child support, you will have to file the name change petition with that court and provide the father notice of your request.

the short answer is yes if you wanted to change the baby's last name you would have to have the biological father's permission. You could use a hyphen after the child's name.
