Name Change for 18 Year Old Child

I have a 18 year old daughter who wishes to change her name to my married name. I was told that all I would have to do is aquire the name change form, purchase it and take it to the courts to have her name changed to--name removed-- After searching the only thing I have found was name change for a minor. She is 18 and I think is considered an adult. Please advise and if possible tell me how I could purchase the correct form to have her name changed. Thanks.
