Changing My Last Name

I want to change my lastname back to my maiden name. How would I go about doing this? Is it something I can do myself or do I need a lawyer for this? What is the fee for this and do I have to change it in my divorce decree as well? I have been divorced for 5 years now. Thank You for your time and help.

You can change your name, by filing a petition, and obtaining a court order. If you know how to proceed, you do not need a lawyer. If not, hire a lawyer in your county of residence.
