Changing Minor Child's Last Name

What steps do I need to take to change my 8 mth old daughters name? Her father and I both agree that we want to hyphenate her last name using my last name and his last name. I just dont even know where to begin the process and how much it will cost.

With the agreement of both parents, this can be a fairly straightforward process. A petition is filed, and an agreed order is submitted to the court. In the event that there is agreement from both biological parents, my fee is $100.00, together with costs (which should be less than $25), and filing fees to the county, which depending on the county are approximately $200.00.
