My ex husband has agreeded to sign a form saying that if something were to happen to me. Our daughter could continue to live with my new husband who supports her now. What type of form should we fill out?

That does not exist as a form, per se. The only alternative I could suggest is that you file a modification, including your husband as a joint conservator. Then, upon your death, he would have standing in court to ask for primary conservatorship. Generally speaking, stepparents do not have rights to the children. If your ex wants to terminate his parental rights, and let the husband adopt her, then it would be problem solved.
