child custody, name changes, ssn changes

Mother and father of child were never legally married, but father is named on birth certificate. Father was extremely violent with mother. There are over 20 police reports documenting this violence and he almost managed to kill mother four days before child was born. Mother left father when child was four months of age. Child has seen father about 10 times in five years. Father has never given any kind of support to mother or child. Mother is forced to keep some contact with Father (email and phone calls) because he threatens to kill her if she does not. Mother would like to legally change both her last name and child's last name as well as both social security numbers. She would like to do this in order to 'disappear' so that Father can never find her or child again. Is there a possible way to do this without having the father authorize the name change on the child? Will the social security numbers be able to change? Can she terminate his parental rights and still be able to disappear?

I don't believe that she could do anything without

notice to the father. The best option is to Terminate

his Parental Rights and then she can disappear. Feel
