Child's Last Name

My Daughter is supposed to have her Dad's last name added to hers since she is under mine. Where can i find the information on how to do this and how much does it usually cost? thank you for your help

Change of name of a minor will require the father's consent, or a hearing. Once the petition is filed, and the father is noticed, either an agreed order is submitted to the court, or a hearing is set. Filing fees vary from county to county, so call the District Clerk for filing fees. Service fees will also vary depending upon the father's location and cooperation.
