Can the custodial parent be held in contempt for not following the judges orders regarding visitation?

Absolutely. It is important to document the events surrounding denied visitation, so that admissible evidence is available at the hearing. When there is a contentious situation, I often suggest that the parent have a third person present as a witness (although it is important that the witness be a neutral party - not a new girl/boyfriend).
