custody form name?

What is the name of the form that needs to be filed when an unwed couple who live together with 2 children are splitting up? I live with the father of my 2 children and I want to leave him. we are not married nor have I ever represented myself as his wife. He does not know I plan to leave with the children and I want to keep it that way. I was told the name of the form I needed to file but have forgotten it. Can someone let me know the acronymn?
