Legal Question in Family Law in Texas

***Please Help With this Custody Issue****

I am recently seeing a man that has had a horrible situation come upon him. His son was 20 days old and he was the last to handle the baby so he was found guilty for injury to a child (even though the mother has a large cps file). He's never had anything on him before and the judge told him he could have been made innocent but because they couldn't figure out what it was that happened to the baby, if he plead innocent the child would be placed in foster care. To stop this from happening he took the guilty plea and received 5 years probation with supervised visitation. He and I have known each other for over a year and been dating a short time and I have seen first hand how his ex works. She changes the visits just to make it as difficult as possible for him to see his son. She continually harasses him and uses their son as a pawn to keep him in some sick way. She continues to play this game and when confronted about it, claims she only contacts him because of his son, which is a lie. What can we do to get it to stop and get his rights completely terminated to stop her? He already signed the papers she got drawn up but she wont file them and uses them as a threat for him to do what she wants, so what can we do to get it done?
