divorce decree

i am doing my own divorce and wanted to know do i put my maiden name on the decree or married name for the section of child custody or any other request on the form.

Most law libraries at the county courthouse will have some book or books on how to do your own divorce. City libraries have the same type of books. A petition for change of name can be joined in a petition for divorce. So if you want to change your married name back to your maiden name, you may petition the Court for that change as part of your divorce.
