Legal Question in Family Law in Texas

Divorce?

My Ex-wife got a divorce from me in April, 2002. I just recently received a copy of the divorce decree Nov. 2003, and I noticed that it stated that we have no children together and are not expecting any, which is not true my son is two years old as of Sept. 2003. The other thing that I noticed was that my signature was signed and notarized which I never saw or signed. The last thing I ever signed for my ex-wife was a letter for Social Security giving permission for my son to receive benefits. What can I do to get changes made to this decree and restore my parental rights?
