Legal Question in Family Law in Texas

After our divorce in 2009, I found out that my ex-husband had a mistress before and during the divorce and he had listed her as his first dependent on his medical insurance paid for by the University of Houston, where he is employed. He failed to disclose his adultery relationship during the divorce and lied about her and giving her checks for $300 from the joint account. What shall I do about all this now?

You don't have any useful legal options. Getting a judge to care about a $150 (half of $300) reimbursement claim from 8 years ago would be impossible and the time for making such a claim has long passed. I'm sorry about the revelations about his girlfriend. I know that must be a painful thing to encounter and endure. Sadly, at some point, the legal system favors finality over moral justice, and your case has passed that point by now.
