Legal Question in Family Law in Texas

Doing my on divorce and I recently turned in my decree approval and it was denied. The judge wrote unable to approve for the following: Family violence has/has not happened.. My question is does that statement have to be in the decree and if so where do I put it.

Put the statement in the decree anywhere that seems appropriate.

It should say, "It has been represented to the Court that there has been no pattern of child neglect or family violence by any party to this case within two years preceding the filing of this case or during the pendency of this case."
