what document would i use for an agreement that the father will not have anything to do with his child? We are not and were never married.

You would have him sign an Affidavit of Relinquishment of Parental Rights. However, this document is used in a court proceeding where a person's parental rights are being terminated (by a judge - a court order). The only way to make it permanent and binding is to file something with the Court.
