In simple terms --

A person who has the authority to make business type decisions is called an attorney-in-fact and has to have been named as such in a Power of Attorney made while the person making the appointment was capable of making decisions for him or herself.

A person who has been given the power to make medical decisions through a Medical Power of Attorney is usually referred to as an agent or a health care agent. The Medical Power of Attorney naming them has to have been made while the person making the appointment was capable of making decisions for him or herself.

If there has been a formal guardianship set up through the courts, the person who makes the health and living arrangement decisions is called the Guardian of the Person. The person who controls the money and the property is call the Guardian of the Estate.
