Legal Question in Family Law in Texas

Ex's address

I have sole custody of my 5 year old son. My ex wife gets visitation every other weekend. I have no idea where she lives, she will not give me address because she says she was not court ordered to do so. She is bipolar and does not take her medication, Please let me know what I can do.

Go back to court, get a modification that orders her to provide this information. Have her served the next time she returns the child.

What does the order say? It's unusual for the standard language not to require each CP to notify the other of their address or any change. If you don't have the language in there, you'll have to get a motion to modify for the language that you want.
