i am fighting to make sure my ex does not get unsupervised visitaion of our daughter when released from state jail, he is a sociopath and has put her in danger before and i feer it will happen and she might not survive. how do i go about proving to a judge he is mentally ill and a danger to our child if he has never physically hurt her, but mentally and emotionally, and has endangered her due to carelessness?

This is not a simple proceeding, to restrict visitation. You will either need to learn a great deal of law, or hire a lawyer, to restrict visitation to a supervised, safe environment. It is not about finding and filing a form. It is about succeeding in an evidentiary hearing, whose outcome directly affects the safety of your child.
