Where can I find forms to file a petion to restrict/ revoke my ex husbands rights & visitation? He owes over $11,000 & is an addict

There is no "form" to restrict or revoke your ex-husband's rights to access or possession.

However, you CAN request the court to (1) enforce the current child support orders and (2) modify the current parenting plan to ensure your children are protected if and when they spend time with your ex (i.e. asking for their visits to be supervised, requesting random drug/UA tests to ensure he stays clean and sober, requesting that your ex be ordered to refrain from any alcohol or drug use within 12 hours before and/or during his possession of the children, just to name a few). You would do this by instituting an Enforcement Action and a Modification Action against your ex-husband.

You can go to any law library and try to do it yourself.

I recently saw one lady at the courthouse that had spent her entire 2 week vacation at the courthouse who was determined to do it herself. She refused to hire an attorney. She was getting a crash course in family law. The judge and the clerks were getting tired of her and her forms -- but she kept trying. The last time I saw her she still had not gotten her divorce but she was still very determined! The judge was still just as determined NOT to sign an improperly prepared divorce decree! She kept complaining to anyone who would listen to her but she did not want any attorney to help her! She was determined to figure it out herself!

You can go to website and buy a kit. However, most I've seen are not worth the postage they cost! Their guarantees are a joke! I have several of these "kits"and have thoroughly enjoyed reading these "kits" Remember, there is a fool born every minute & you get what you pay for!

I highly recommend that you hire an experienced family law attorney and do it right the first time.

What you are asking for is complex and requires an attorney with experience.
