where can i find a motion for judge to confer with child?

Probably at the county law library. Do not pay for a form on-line, they are almost always worthless.

Worst case, just put the case number and heading on the top of a paper (just like all the other pleadings in the case) and write something like this:

Pursuant to Texas Family Code section 153.009, , requests that the Court interview the child
