Almost every courthouse I know of, has this form in the District Clerk's office, and the courthouse library. Almost every courthouse also has a notary that will sign it for you, usually for $10-$15 fee. Some county law libraries have them available online. If you need to rewrite the form to the specifications of your case, then rewrite it.
