What forms do I need to complete if I want to file contempt of court on my ex husband? He went behind back, took our daughter out of school and has her being home schooled with a relative. All without my knowledge. Although she was residing with him, the final divorce decree states that we have joint custody of her and I am to be made aware of everything. He definitely violated the decree. I need to know how to file without an attorney.

You need an attorney. This is a complex matter.
You can go the Houston Law Library in downtown Houston and try to figure it out. They have a copy of the Texas Family Practice Manual. Contempt is technical and if you don't do it exactly the right way, an attorney an ask that it be thrown out of court.
Read your decree and see if you are required to attend mediation before filing anything.
