Legal Question in Family Law in Texas

I need a FREE form to fill out for a petition to file for a default judgement on child custody to REMOVE ALL RIGHT EVERY SINGLE ONE INCLUDING PROVIDING SUPPORT... since the AG in TEXAS has spent 2 1/2 yrs hunting for a stupid DNA sample even though he's worked for the same employer 4 yrs and I gave them the addresses and info and he's been in JAIL FOUR TIMES and each time I contact the AG letting the KNOW HE WAS IN JAIL AND WHAT COUNTY AND WHAT CHARGE

No such thing. You need to hire a family law attorney. Terminating parental rights is much more involved than just filling out a form--at any cost.
