My god daughter currently resides with me in the state of Texas. She is 14 years old and intends on finishing school here. Her mother is in NY and has given me a notarized letter so that I may enroll her in school and take care of her needs. I am struggling with insurance as my job will not honor the document I have. They state it must come from the court. I have seen a few forms online where there is court appointed guardianship which would be easier for her mom and myself so we can void ourselves of the letter she writes every year. I would like advice on how to proceed and in which state should such a form be completed. TX or NY

I think that a Chapter 34 Non-Parent Relative Authorization form could work for you IF you are related to the child. If you are not related to the child, then you can file a friendly suit affecting the parent-child relationship suit and get a court order that requires you to purchase health insurance, which will definitely work. Don't go down the guardianship route. Too complicated and will annoy you for years to come.
